Application Procedures for Disclosure of Personal Information Subject to Disclosure in the Company’s Possession
To make a request to the Company pursuant to the JIS Q1005:2006 for notice of the purpose of use, disclosure, correction, supplementation, deletion, suspension of use, deletion, or suspension of provision to third parties (referred to as “Disclosure etc.”) with regard to Personal Information Subject to Disclosure in the Company’s possession, please download the application form designated in A below, fill in the necessary information, enclose the identification documents specified in B below, and mail it to the address indicated at the bottom of this document. Submission of applications is limited to mail, and applications submitted by telephone, facsimile, email, or otherwise cannot be accepted.
A. Designated Application Form
Form IS05-1: Request for Disclosure etc. of Personal Information (PDF)
B. Identification documents
(1) Identification with a photo (a copy of any one of the following)
a. Driver’s license
b. Passport
c. My Number Card
d. Student ID with photo
(2) If you do not have identification with a photo (copies of two or more of the following)
a. Certificate for public health insurance
b. Pension book
c. Maternal and child health handbook
d. Student ID (without photo)
The Company will respond in accordance with JIS Q1005:2006 to requests from the legal guardians of minors and adult wards and agents appointed by the individual. In this case, in addition to the application form specified above, enclose the following.
(1) In the case of a legal guardian (all of the following are required)
a. Required information relating to legal guardians at the bottom of Form IS05-1: Request for Disclosure etc. of Personal Information (PDF).
b. A copy of a certificate of registration verifying the legal guardian (in the case of parental rights, a copy of a health insurance card with the name of the dependent is acceptable)
c. Documents confirming the identity of the legal guardian (the identification documents specified in B above may be used)
(2) In the case of an appointed agent (all of the following are required)
a. Power of attorney written by the individual
b. Documents confirming the identity of the agent (one of the identification documents specified in B above may be used)
Requests for notification of the purpose of use or disclosure of personal information are subject to a fee of 1,000 yen (including consumption taxes) per request. We will contact you after receipt of the application form regarding the method of payment. If you do not make payment using the specified method by the last day of the month after the month in which notice is provided, the request for Disclosure etc. will be considered not to have been submitted.
Responses to requests for Disclosure etc. will be sent in writing to the applicant’s address indicated on the application form by certified mail that may be received only by the addressee.
Personal information that the Company obtains in relation to requests for Disclosure etc. of Personal Information Subject to Disclosure shall be handled only to the extent necessary to process the application.
Please note that the Company is not able to respond to requests for Disclosure etc. in the following cases.
a. Matters relating to applications in general
In any of the following cases:
- If we are unable to confirm the applicant’s identity such as in the case where the address indicated as the address confirmation on the application form does not agree with the address registered with the Company.
- The case of an application by an agent, if we are unable to confirm that the agent’s rights.
- If the designated application form is not properly completed.
- If the scope of the application is not Personal Information Subject to Disclosure covered by Disclosure etc. recognized by JIS Q1005:2006.
b. Request for disclosure of purpose of use
In any of the following cases:
- If notice or release of the purpose of use to the individual would pose a risk of harm to the life, person, property, or other rights and interests of the individual or third parties.
- Notice or release of the purpose of use to the individual would pose a risk of infringing on the Company’s rights or legitimate interests.
- In the case where cooperation with the implementation of administration specified in laws and regulations by a national agency or local governmental body is required, if notice or release of the purpose of use to the individual would pose a risk of interfering with the implementation of such administration.
- If the Company has previously publicly announced the purpose of use of the relevant personal data.
c. Requests for disclosure, suspension of use, deletion, or suspension of provision to third parties
- If there is a risk of harming the place, person, property, or other rights and interests of the individual or third parties.
- If there is a risk of substantial interference with the proper implementation of the Company’s business operations.
- If such action would violate laws and regulations.
